Assessment
NCLEX-RN glossary · Physiological Adaptation
Definition
The first layer of the Clinical Judgment Cycle; it involves the deliberate collection of objective and subjective cues to build an evidence base.
Assessment in plain English
Failures in this step (missing a cue) invalidate the entire judgment cycle and lead to inappropriate nursing actions. The main trap is selecting a derivative action (intervention) before re-assessing a baseline cue that may have shifted after an intervening event.
