Documenting before administration
NCLEX-RN glossary · Pharmacological And Parenteral Therapies
Definition
The unsafe practice of recording a medication as given before it has actually been received by the client.
Documenting before administration in plain English
It creates a false medical record and risks drug omissions if the RN is interrupted before the dose is delivered. A frequent error is pre-charting a medication pass to manage time during a busy shift, leading to documentation of care that has not occurred.
