Mixing incompatible insulins
NCLEX-RN glossary · Pharmacological And Parenteral Therapies
Definition
The incorrect combination of different insulin formulations in one syringe, which can alter pharmacokinetics or lead to drug precipitation.
Mixing incompatible insulins in plain English
Considered a high-alert medication error; incorrect mixing can result in unpredictable glycemic control, hypoglycemia, or loss of therapeutic effect. A common mistake is contaminating a vial by drawing intermediate-acting (cloudy) insulin before short-acting (clear) insulin, violating the 'clear-to-cloudy' safety protocol.
