One unstable client among stable assignments
NCLEX-RN glossary · Management Of Care
Definition
A prioritization task where the nurse must distinguish a single client with new, subtle cues of instability (e.g., restlessness or tachypnea) from a group of clients with chronic, expected findings.
One unstable client among stable assignments in plain English
Failure to identify the unstable client leads to a delay in rescue; it is the cornerstone of effective multi-client prioritization and failure-to-rescue prevention. The main trap is prioritizing a client with high-intensity but stable needs, such as severe chronic pain or a scheduled discharge, over a client with a quiet, new cue of physiological decline.
