Pain score without functional assessment
NCLEX-RN glossary · Basic Care And Comfort
Definition
The practice of recording a 0-10 pain value without evaluating the client's ability to perform activities of daily living, deep breathe, or participate in mobility.
Pain score without functional assessment in plain English
This is a numerical score alone is insufficient for judgment; interventions must be based on how pain interferes with the client's functional goals and quality-of-life surveillance. A frequent error is the misconception that a high numerical pain score automatically mandates a maximum dose of medication, regardless of the client's sedation level or functional capacity.
