Pressure injury prevention

NCLEX-RN glossary · Physiological Adaptation

Definition

A set of proactive nursing interventions, such as scheduled offloading and moisture control, that maintain skin integrity. It defines the minimum standard of care for any immobile client.

Pressure injury prevention in plain English

It prevents the accumulation of 'physiological debt' caused by secondary infections and chronic wound healing processes. The main trap is prioritizing advanced wound dressings over the foundational nursing priority of frequent repositioning to offload pressure.
