A patient is unable to reabsorb glucose in their proximal convoluted tubule, resulting in glucose in the urine (glycosuria). Physiologically, this most likely occurs because:

  1. Antidiuretic Hormone (ADH) is inhibiting the reabsorption of sugar
  2. The loop of Henle is not creating a sufficient osmotic gradient
  3. The glomerulus has become too permeable, allowing too much glucose to be filtered
  4. The blood glucose concentration has exceeded the transport maximum (T_m) of the carrier proteins
