Percent change
NCLEX-RN glossary · Reduction Of Risk Potential
Definition
A quantitative tool for trend reasoning calculated by taking the difference between a new value and an old value, dividing by the old value, and multiplying by 100 to determine relative clinical movement.
Percent change in plain English
This is essential for identifying the trajectory of decline before absolute thresholds are reached, forcing the nurse to act on the rate of change rather than a single static measurement. The main trap is evaluating severity based solely on the raw numerical difference between values, which often obscures the physiological significance of the change for a specific client's baseline.
